What to Do as a New Patient

  • Fill out new patient information online (just click the tab below).

    Call the office at 208-291-5553 to verify insurance and request an appointment.

  • Once we have you scheduled for your first appointment, you will receive the patient registration link, to your email on file, to register for the patient portal. 

    • Please be aware that the patient portal link is good for 24 hours. If it requires more time, you will need to request your portal link to be resent.

    • If you are a parent signing up for your child(ren), you may use the same email address for multiple patients, but each will need a new username for the login.

  • Once registered for the patient portal:

    • Sign all consents

    • Verify your information

    • Upload a copy of insurance information, or card, to your patient portal documents.  (This is needed for diagnostic and lab orders.) 

    CONSENTS will need to be filled out prior to your first appointment.

  • You will receive an email and text message with appointment details.
